Specialist Support Coordination (SSC)
Our Specialist Support Coordination team works creatively and resourcefully with NDIS participants to support them to utilise their plan and achieved their goals.
We offer two levels of Support Coordination at Rehability Australia:
Specialist Support Coordination - SSC (Level 3)– This is a higher level of support coordination. It is for people whose situations are more complex and who need specialist support. A specialist Support Coordinator will assist you to manage challenges in your support environment and ensuring consistent delivery of service.
Depending on your individual goals, plan objectives and aspirations you may receive funding for these supports in your plan.
Our Support Coordinators can assist participants in the following:
- Understanding and rolling out your plan
- Building capacity
- Finding services and supports
- Navigating systems
- Assisting to manage crisis
Support Coordination
Our Support Coordination team works creatively and resourcefully with NDIS participants to support them to utilise their plan and achieved their goals.

Support coordination – coordination of supports (Level 2): This support will assist you to build the skills you need to understand, implement and use your plan. A support coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.
Depending on your individual goals, plan objectives and aspirations you may receive funding for these supports in your plan.
Our Support Coordinators can assist participants in the following:
- Understanding and rolling out your plan

- Finding services and supports
- Navigating systems
- Assisting to manage crisis
Navigation
Our highly trained Support Coordination team offers specialised navigation support, to assist participants navigate service complexity
This specialised support intervention is to address challenges with navigating service and the support sectors.
Rehability Australia offers specialist skills in navigation support including assessment and the development of a comprehensive plan that aims to reduce and manage challenges associated with navigating the disability service sector.
We have a team of registered Navigation Practitioners to assist you with your needs.
Our Navigation Practitioners can assist in the following:
- General Navigation
- Specialist Navigation
- Psycho-social Navigation
- Housing and Living Navigation
