Social Work

Clinical social work services, such as counselling, therapeutic support, capacity building, community participation and accommodation assistance.

Social workers are trained allied health professionals who are skilled in addressing the psychosocial needs of individuals. Psychosocial assessments looks at the individual’s situation in the context of their psychological and social functioning. It considers a multitude of factors including physical, functional, psychological, social situation as well as personal, family, relationships, and practical matters such as housing, finances, legal, community participation and occupational needs.

Social workers provide a unique role in that they not only focus on facilitating social change and development, social cohesion, and the empowerment and liberation for individuals through skill/capacity building, but unlike other professions, social workers have a dual focus on also identifying, addressing and navigating any external issues (known as system or structural issues) that may create inequality, injustice and discrimination, or may impact on wellbeing or an individual ability to achieve their identified goals.

Case Management

Our Social Work Case Managers offer a collaborative process of assessment, planning, facilitation, monitoring and review to achieve client-centered goals and meet the holistic needs of an individual.

The case management role navigates each phase of the case management process (as applicable) with careful consideration of a person’s individual, diverse and special needs. The case manager considers the individuals aspirations, choices, expectations, motivations, preferences and values, and available resources, services and supports.

Rehability Australia offers specialist Social Work services to individuals who work under NIISQ, I-care and TAC Schemes in accordance with the local state based legislation, to ensure individuals who suffer serious injury as a result of an accident are provided with rehabilitation and support to maximize independence, increase participation in the community, engage in employment and to meet their psychosocial needs.

Behaviour Supports

Our highly trained team offers positive behaviour support through our assessment and behaviour management service.

This highly specialised intensive support intervention is to address significant harmful or persistent behaviours of concern.

Rehability Australia offers specialist skills in positive behaviour support including assessment and the development of a comprehensive plan that aims to reduce and manage behaviours of concern. This may include training for carers and others in behaviour management strategies required due the persons disability.

We have a team of registered and accredited Behaviour Support Practitioners to assist you with your needs.

Our Behaviour Support Practitioners can assist in the following:
  • Identifying behaviours of concern
  • Assessment of behaviours of concern
  • Completion of functional behaviour assessments
  • Development of positive behaviour support plans
  • Management of restrictive practices
  • Training in behaviour support strategies
