NDIS Pre-Planning Assessment and Support Service
Rehability is a specialist rehabilitation and disability service, specialising in providing quality services to individuals within the NDIS, NIIS, Defence/DVA and the private sector. Rehability is registered to provide supports for the National Disability Insurance Scheme participants across the eastern states of Australia. We are emerging as a market leader providing support and guidance for both individuals who wish to enter NDIS and also individuals already in the NDIS. We hold an intimate knowledge of the NDIS, working within an Insurance framework in addition to having over 15 years as specialist providers for people living with acquired disabilities. Our experience and knowledge positions us to be able to efficiently and effectively assist individuals in understanding and entering the NDIS scheme.
Services: Pre- Planning Assessment and Support Service: Support and assistance to address individuals pre-planning needs by offering a comprehensive client review, complete a needs assessment and development of detailed report in preparation for NDIS.
Assisting individuals to register for NDIS, supporting individuals and/or attending on behalf of individuals at NDIS planning meeting to facilitate development of an individual’s ‘My Plan’ and negotiating any compensation reduction amount (if applicable) and the impact this may have on services and supports.
Annual review: Completion of an annual review: aimed at re-assessment of the individual’s needs and progress towards identified ‘My Plan” goals and development of new/updated ‘My Plan’ for the subsequent 12 month program period. Supporting individuals and/or attending on behalf of individuals at NDIS planning review meeting.
